What is the AI Readiness Self-Assessment?

The AI Readiness Self-Assessment is a diagnostic tool designed for healthcare, state, local government, and community-based leadership teams. It measures current operational and technical capabilities across five critical domains:

  1. Agency Strategy: Leadership alignment, strategic roadmaps and funding commitment
  2. Technology Strategy: Data quality, interoperability and integration, infrastructure, and technical safeguards.
  3. Applied AI Experience: Workflow readiness, pilot evaluation, and use case prioritization.
  4. Organization and Culture: Workforce training, change management, and staff engagement.
  5. AI Governance: Regulatory and policy compliance, privacy and security safeguards, AI risk evaluation, accountability, and oversight structures.

Why Complete This Assessment?

Whether your team is evaluating initial AI concepts or scaling active deployments, establishing an accurate baseline provides clear strategic value:

  • Establish an Evidence-Informed Baseline: Document current capabilities using verifiable organizational practices.
  • Target Strategic Growth: Identify key focus areas spanning data architecture, workforce adoption and compliance.
  • Measure Progress Over Time: Complete the initial baseline now, then retake the evaluation at the end of the learning series to identify changes in organizational readiness over time.

Instructions and Completion Guidance

To ensure assessment results remain reliable and actionable, keep these guidelines in mind before submitting your evaluation:

  1. Base Responses on Present Reality: Select ratings based on existing documentation, active governance structures, assigned ownership, or operational evidence today.
  2. Consult Others as Needed: If possible, complete the assessment with input from colleagues across leadership, technology, operations, compliance, privacy, security, and workforce development.
  3. Select Unsure When Appropriate: Accurate baseline data is critical. Choose 3 – Neutral / Unsure or Not Sure rather than estimating capability levels.
  4. Estimated Time: Completing this evaluation takes approximately 10 to 15 minutes.
