A collaborative and creative healthcare professional, Leslie Brooks partners with health plans and providers to design, implement, and evaluate integrated models of care in safety net populations, emphasizing population health management and measurement-based care. Her model expertise includes Collaborative Care, Complex Care Management, and Health Homes models of care.
With more than a decade of experience, Leslie is skilled at utilizing data to drive practice change and improve quality of care. She works with multidisciplinary stakeholders to bring health information technology to the front lines of the delivery system. She has experience implementing clinical registries, care management platforms, and dashboards and reports to drive improvements in patient care and population health management.
Since joining Health Management Associates (HMA), her project work has included implementing large-scale population health management, Health Homes, and Enhanced Care Management initiatives, developing care management tools, and helping practices prepare for a transition to value-based payment through model of care redesign. Most recently, Leslie partnered with the Population Health Learning Center and the California Department of Health Care Services to develop and deploy the Equity and Practice Transformation Program, a three-year direct payment program, to over 200 primary care practices in California with the goal of improving population health capabilities, reducing health inequities, and achieving improvement in health outcomes for Medicaid enrollees. Additionally, she recently served as the project manager for a large-scale population health initiative involving 32 community health centers (CHCs), for which she also provided direct training and technical assistance on empanelment.
Before joining HMA, Leslie served as a manager of population health and worked with a California-based Medicaid managed care plan to implement a behavioral health integration and complex care management program across 12 diverse healthcare organizations and 31 care teams in south central California. In this role, she developed and implemented a clinical registry with the participating care teams and provided training and ongoing technical assistance. She partnered with practice coaches to implement key practice changes, including measurement-based care, and coordinated evaluation activities and the analysis of clinical outcomes to ensure continuity in how data was defined and used across the project.
In her work with the managed care plan, Leslie also served as an advisor for the plan’s population health strategies, with an emphasis on data integration, and collaborated with Healthcare Informatics and Information Technology departments. Prior to this, she also worked as a health plan analyst and the system administrator for a web-based patient registry.
Leslie earned a bachelor’s degree in English literature from the University of Washington, Seattle.
