A collaborative and creative healthcare professional, Leslie Brooks focuses on collaborating with health plans and healthcare teams to implement whole-person, person-centered approaches to healthcare integration, complex care management, and health home approaches.
With HMA, her project work has included implementing a behavioral health integration and complex care management program across 12 diverse healthcare organizations and 31 care teams in health plans in California. This work prepared teams for participation in the California State Health Homes Program.
Leslie’s work has also focused on utilizing data to inform care and understand healthcare impacts across the four domains of the quadruple aim, with an emphasis on using clinical registries to drive individual patient care and population health management.
Before joining HMA, she served as a of population health manager with a health plan where she provided training and technical assistance for data analysts, clinical teams, and healthcare organizational leaders. She also developed and coordinated evaluation activities and informed health plan population health strategies focusing on data integration and collaborating with health plans to lead practice transformation, healthcare informatics, and information technology use.
Leslie also has experience as a health plan analyst and in business development, managing relationships, administering affiliate programs, and partnering with information technology and marketing departments.
She earned a bachelor’s degree in English literature from the University of Washington, Seattle.
