Helping ACOs and Community Partners succeed in MassHealth transformation.

Welcome to HMA’s MassHealth ACO and Community Partner technical assistance page. We were selected for all nine technical assistance domains for ACOs and Community Partners.

HMA is already working with ACOs and Community Partner (CPs) clients to help them succeed under the new MassHealth model. In addition, HMA is leading learning collaboratives for the Massachusetts League of Community Health Centers to help CHCs succeed in the new value-based payment environment. We are supporting primary care and behavioral health integration through the Health Policy Commission’s PCMH PRIME program. And we are helping ACOs and CPs learn how to work together toward common goals.

Below you can find information about each of the domains of technical assistance HMA can provide, including HMA experts, our experience, and our approach to ACO and CP work. HMA project teams will be led by Boston staff and will draw upon our experts from across the country to provide exceptional technical assistance to Accountable Care Organizations and Community Partners alike.

HMA understands the importance of population-based, person-centered systems of care. We have developed analytics and interventions to address social determinants of health, including an innovative framework to examine health disparities in the Medicaid population based on demographic, medical, and social risk factors. We have partnered with consumers having diverse backgrounds and complex needs to design and implement projects, and we know that engaging consumer stakeholders is vital. HMA is committed to improving care for MassHealth members, and we know that commitment is also the core mission of ACOs and CPs.
