An increasing focus on population health has drawn attention to the integration of behavioral health—mental health and substance use/abuse—with physical health as a way to reduce costs and improve quality of care.
Our behavioral health team has the knowledge and expertise to advance integrated care from concept to results. We’ve developed evidence-based models that can work at any level of your organization. Our behavioral and primary care expertise includes:
- Conducting readiness assessments for implementation of integration models, including assessing current workflow, analyzing utilization and outcomes data, identifying gaps and opportunities and creating action plans and timelines
- Providing project management and expertise to lead your organization through the integration process, including integrating behavioral health and primary care and implementing community integration models including Community Health Workers (CHWs), behavioral health home models and centralized care management models
- Holding one- and two-day care management training workshops for behavioral healthcare managers on topics including team care models for behavioral health integration; understanding behavioral health conditions; care management soft skills such as motivational interviewing, self-management and shared decision making; and behavioral healthcare technologies including registries and population health
- Conducting integrated team model training on topics including whole-team learning, team roles and efficiencies, team workflow and hand-off techniques, building trust and effective communication skills
- Providing consulting on financial strategies that support integration
- Helping organizations navigate federal, state and private funding opportunities to secure support in integrated systems of care
