Federal policymakers consider current and future spending measures on simultaneous tracks

This week, our In Focus section covers Congress’s and the Administration’s parallel efforts to finalize fiscal year (FY) 2024 spending bills and begin the budget process for FY 2025.  

Congress approved a bipartisan package for some of the FY 2024 spending bills, and on March 9, 2024, President Biden signed the Consolidated Appropriations Act of 2024 into law (PL 118-42). Programs funded through this measure include the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) and other federal nutrition supports, rental assistance for safe and affordable housing, and veterans medical care and benefits.  

Several mandatory funding extensions of public health programs and health-related policies also found their way into the 2024 consolidated appropriations package, including extending the Community Health Center Fund, delaying reductions in the disproportionate share hospital allotments, defining Certified Community Behavioral Health Centers (CCBHCs) as a Medicaid service, extending incentive payments for certain Medicare providers, and mitigating the impact of cuts to the Medicare physician fee schedule.  

These policies, however, addressed a narrower set of issues than the expansive and bipartisan legislation that has been moving through both chambers of Congress. For example, House and Senate members have worked on respective bipartisan policies affecting price transparency, pharmacy benefit managers, and Medicare site-neutral policies, among others.  

Meanwhile, President Biden released the FY 2025 Budget proposal March 11, 2024, kicking off the annual budget process. Like the administration’s FY 2024 budget proposal, the FY 2025 plan emphasizes deficit reduction and continues to make equity and Medicare solvency cornerstones of the budget. Health-related priorities include expanding access to affordable healthcare services, lowering drug costs, improving maternal health, addressing the mental health and substance use disorder crises, and enhancing biodefense and preparedness activities.

What We’re Watching 

Congress is continuing negotiations on the outstanding spending bills, including the one that funds the Departments of Health and Human Services, Labor, and Education through September 2024. Lawmakers are working to reach an agreement before the next funding deadline of March 22.  

The administration’s FY 2025 budget proposals are generally being characterized as a blueprint for President Biden’s re-election campaign and, if successful, a policy agenda for his second term. Though Congress has already begun holding hearings on the budget request, members on both sides of the aisle will likely focus on issues that resonate in an election year.  

Regardless of the outcome of the November elections, Congress has an opportunity to address unfinished business during the lame duck session later this year.

Focus on equitable access at 2023 HMA conference

Access to care is not as simple as obtaining an insurance card. Some people find access to care is limited by geography or distance, others are limited by their native language or cultural awareness. In other ways, care can be limited by who is in the insurance network. All of these inequities can cause gaps in care that undermine health outcomes. In a system that is increasingly paying for outcomes, elimination of inequities is a matter of financial performance, as well as a sign of clinical excellence. Finding and reducing inequities in access to care requires an operational commitment to change workflows, leverage technology, and train staff at all levels to align incentives and culture.

Providing equitable access to care is subject to ever-changing policy and regulatory requirements, and it is increasingly tied to funding, work force staffing and many other operational requirements. This topic will thread through several discussions and panels during the 2023 HMA fall conference with federal policy leaders, health system administrators, and other industry leaders all poised to address the pain points of achieving and maintaining equitable access.

  • Understanding and Meeting New Health-Related Social Needs Requirements – An environmental overview, including a look at what’s driving these demands and how organizations are specifically working to address the new mandates. (Monday 1:30pm breakout session featuring Bryan Buckley of NCQA, Richard Ayoub of Project Angel Food, and Paul Leon of National Healthcare & Housing Advisors)
  • Practical Approaches to Ensuring Equity in Publicly Sponsored Healthcare Programs This session will provide practical approaches to addressing equity, including an overview of efforts by policymakers, health plans, and providers to make equity the central component of all initiatives to improve healthcare outcomes, access, and health-related social needs. (Tuesday 8:30am keynote by Karen Dale of Amerihealth Caritas)
  • Medicaid in a Post-Pandemic World: Challenges, Opportunities, and a Renewed Focus on Equity State Medicaid directors will provide a status report on all this and more, including a special emphasis on how equity plays into planning and policy decisions. (Tuesday 9:15am plenary featuring Jacey Cooper of the California Department of Health Care Services, Kelly Cunningham of the Illinois Department of Healthcare and Family Services, Drew Snyder of the Mississippi Division of Medicaid, and Stacie Weeks of the Nevada Department of Health and Human Services)

Advancing the national dialogue on improving access to quality care

Healthcare executives gathered with HMA leaders in March to learn and share about new initiatives in quality improvement. Panels and discussions were led by esteemed experts, who provided important insight into the multitude of opportunities to improve quality and equity:

From the Experts: CMS has doubled down on its commitment to improve healthcare quality, equity, and access.

The HMA Point of View: Current leadership in Washington has set very clear goals to improve health equity, aligning federal policy as a lever to improve healthcare outcomes. Federal dollars passed along to states are including new rules and objectives to improve quality in an equitable manner. Therefore, those applying for federal contracts and grants must include a strategy to improve quality, equity, affordability, and access.

From the Experts: Interventions need to be robustly evaluated to address equity and social determinants of health (SDOH).

The HMA Point of View: Governments, care providers, and payers have been experimenting with approaches to SDOH, but the bar is now higher and results have to be measurable. New investments that address SDOH and aim to improve equity must include a strategy to measure and analyze results of interventions, including evaluation of critical subpopulations to address disparities. Although experiments will continue, we have reached the point where a robust analysis of outcomes is an expectation.

From the Experts: Quality is playing a central role in operationalizing equity.

The HMA Point of View: While there are many reasons for disparities in health, quality metrics and programs must be designed to improve quality for all patients. Identifying inequities is only the first step; successful programs will advance equity by reducing barriers to care. Measurable quality programs should be designed for population health, but personalized for individuals with clear provider incentives to not only identify disparities but also minimize them. Quality is the tool by which we achieve health equity.

From the Experts: We are on the precipice of doing great things in quality.

The HMA Point of View: Our ability to integrate data from multiple sources is finally getting to a place where patients can get the quality care they need, and providers can give the right care at the right time to improve outcomes. Policymakers have broken down barriers to data sharing, enabling a new economy of information sharing that promises to empower patients and reduce costs. It is no longer enough to have a separate IT or data strategy; anyone working to improve healthcare quality needs to have data strategies within and across every operational function across their organization.

From the Experts: Being rewarded for doing what’s right for the patient is central to continuous quality improvement.

The HMA Point of View: Quality care for the patient means creating the right incentives for payment and care delivery that focus on outcomes and experience. Every point of care in the patient’s health journey needs to be evaluated and designed from the point of view of providing a quality experience. Not every patient has the same journey, and our healthcare system must meet them where they are, delivering for their unique needs. Convenience, personalization, accuracy, simplification, and affordability are nearly as important as clinical outcomes in the mind of most patients and addressing barriers to health can have clinical benefits.

HMA has a long history of working with clients to achieve their quality improvement goals including securing accreditation for both payer and provider organizations, driving clinical practice transformation, and improving the overall value of care. We strongly believe that there is no quality without equity, access, and measurement, and are investing in people and resources to support these needs. Together we are working with clients to advance value-based care, develop programs demonstrating quality, value, and equity in behavioral health, and implement new quality and accreditation programs to better serve their communities.
