For practices seeking to apply for Patient-Centered Medical Home (PCMH) recognition, download helpful information and resources via our newest online resource, “Achieving NCQA PCMH Recognition: A toolkit for practices seeking to apply.” Jointly developed by a team of NCQA PCMH Certified Content Experts from HMA and the Florida and Illinois CHIPRA Quality Demonstration Grant teams, the Toolkit provides clients with an understanding of the resources and effort necessary for practices to achieve PCMH recognition, identifies and provides key resources/tools related to transformation areas and processes that are the most challenging for individual practices, and also serves to inform the medical community and federal and state policy makers of needed resources.
